All our Medicare Advantage plans include prescription drug coverage. We offer both HMO and PPO plans allowing you the flexibility to choose the type network and cost-sharing (e.g., premiums, copayments and coinsurance) that best meets your needs.

In addition, most of our plans include coverage for preventive and comprehensive dental, eye exams and eyeglasses and hearing exams and hearing aids. And all plans include coverage for emergency care when you travel.

Plans are available in select counties in Colorado, Georgia, Illinois, North Carolina, South Carolina, and Virginia.

Special Needs Plans (SNPs) provide personalized guidance and resources to help members get the right care and information based on their specific condition or needs. To be eligible for an SNP, you must have Medicare Parts A and B and meet the specific conditions of the SNP.

Plans may offer extra benefits tailored to the groups it serves, such as diabetes services, care coordination or other health and wellness programs.

Clear Spring Health offers two types of SNPs: Chronic Care (or C-SNP) and Dual Eligible (D-SNP) in Georgia and South Carolina.

CSNPs are for individuals who have been diagnosed with one of the following chronic conditions:

  • Diabetes Type 1 or Type 2
  • Congestive Heart Failure (CHF)
  • Cardiovascular Disease (CVD), who may be in a traditional MA-PD or Original Medicare Plan.

Dual-Eligible SNPs are for individuals who are entitled to Medicare and who are also eligible for assistance from a state Medicaid program including:

  • Qualifying Individual (QI)
  • Qualified Disabled & Working Individuals (QDWI)
  • Qualified Medicare Beneficiary (QMB)
  • Qualified Medicare Beneficiary Plus (QMB+)
  • Specified Low-Income Medicare Beneficiary Plus (SLMB+) are covered by the State Medicaid program for their Medicare cost sharing

Adding a stand-alone Prescription Drug Plan (PDP) to Original Medicare or a Medicare Supplement plan offers an added layer of protection against high healthcare costs. Clear Spring offers two plans to choose from – Premier and Value – in over 40 states.

Premier – Targeted to members looking for a plan with an affordable premium that also features $0 or low copays for generic drugs and low prescription drug usage.

Value – Members who want the security of our most comprehensive plan, with low copays and deductibles.
